Our specialist nursing and personal care services are here to help.
Aurora Community understands that wellbeing and independence is important to veterans, war widows and widowers. If you need support to maintain optimal good health, whilst living independently in your own home, our specialist nursing and personal care services are here to help.
Once you have been referred our community nursing team will meet with you to talk about the support you require and develop a nursing care plan tailored to your individual needs. Your care plan may include things such as:
- Personal care
- Wound care management
- Continence management
- Health checks e.g. blood pressure, pulse, temperature and blood sugar levels
- Medication management
- Palliative care and pain management
Our caring support team includes registered nurses, enrolled nurses and nursing support workers.
Eligibility and referrals
Gold Card holders or White Card holders with an accepted war or service caused injury or disease can access this service. Your general practitioner or a hospital discharge planner can refer you for this service.
There is no fee for this service.
