Accessing Home Care
Four steps to accessing the support you need.
Step 1:
Call Aurora Community (Hunter Nursing and Mayo Home Nursing) on 1800 319 301 and tell us what you need help with. We’ll explain our services and how we can assist.
Step 2:
If you already have an allocated package let us know how you’d like to use your subsidy. If you don’t we can help you register with My Aged Care. You will have a free assessment with your local Aged Care Assessment Team/Service to determine your needs and eligibility.
Step 3:
We’ll meet with you and your carer or family if you wish, to talk about the level of care you need. We can do this in the comfort and privacy of your own home or wherever it is most convenient for you.
Step 4:
We’ll coordinate a personalised home and health care plan.
We understand that the level of care you need may change over time. We can adjust your personalised home and health care plan when you need to.
